We would like you to try to use the online portal to schedule and request and grooming or boarding appointments the QR code is located on the front page.

 Before filling out the below request form, CALL YOUR VET OFFICE and have them send us the following information to records@pethavenpets.com  Vaccinations MUST be current through your boarding/grooming requested dates. If we receive vaccinations that are not current we will NOT follow up with you. We are trying to have all our clients use our new Client portal. Please find the link on our Home Page.   We Need the following

1. Vet signed rabies certificate or signed boarding certificate

2. proof that distemper parvo vaccine is current during you stay

3. proof that Bordetella (kennel cough) vaccines is current during your stay

4. We will also need a negative fecal. This can be sent after we have set up the appointment BUT MUST BE Received no later than ONE WEEK BEFORE your drop off date. Please note it has been busy at the labs for the process of fecal results so please get your sample to your vet as soon as you can. 

5.  NOW THAT YOU HAVE COMPLETED AND UNDERSTAND THE ABOVE STEPS. 

• BY COMPLETING THE BELOW CORROSPONDING FORM I UNDERSTAND AND HAVE COMPLETED THE ABOVE REQUIREMENTS. 

• PLEASE NOTE WE WILL NOT FOLLOW UP UNTIL WE HAVE VACCINATIONS RECORDS IN THE OFFICE. THANK YOU FOR YOUR UNDERSTANDING.
