Vaccination Requirements

Please have your veterinarian forward record ASAP or at least 1 WEEK before your scheduled appointment to records@pethavenpets.com. IF we do not have vaccinations we can cancel your grooming appointment. Thank you Management.

For Boarding Dogs

Rabies- A veterinarian Signed Rabies Certificate

Distemper parvo

Bordetella

Fecal Sample- a negative fecal sample done within in the last 6 months

If you pet is on deworming treatments they will not be allowed out on grass during their stay.

For Grooming Dogs

Rabies- A veterinarian Signed Rabies Certificate

Distemper parvo

Bordetella

For Boarding or Grooming of Cats

Rabies- A veterinarian Signed Rabies Certificate

Distemper

**Records must come from a licensed Veterinarian office.

AGAIN: Please have your veterinarian forward record at least 1 WEEK before your scheduled appointment. For all grooming appointments we will need all vaccinations before an appointment will be made for new clients.

**If records are not received prior to 1 WEEK before your appointment this could cause your appointment to be moved back or possible cancelled until records are received.
